In 2000, Health Outreach Partners (HOP) developed its Farmworker Outreach Program Guidelines based on 30 years of experience providing direct outreach services to migrant and seasonal farmworkers. Description: As health centers are held accountable for cost containment and high quality of care under value-based pay arrangements, it is increasingly important for providers to document the social determinants of health (SDoH) barriers of their patients and what services they are providing (both clinical and non-clinical) in order to properly care for those complex patients. Some of these services are called enabling services (ES), defined as non-clinical services that aim to increase access to care and improve health outcomes.
